Clinical Case 20081123 - General overview


  • Patient: Female, 22 years old.
  • Clinical symptoms: Referred to the hospital due to abdominal pain, diarrhoea and fever, started 1 month ago.
  • Normal abdominal examination except for some tenderness in righ low quadrant.
  • Abdominal examination was normal, except for some tenderness in the RL quadrant.

 

Ileitis

 

Lineal high frecuency probe.  Near the ascendent colon and anterior to illiac artery a tubular structure is seen. It is a bowel loop with concentric wall thickness higher than 4 mm. No peristalsis is seen and the lumen is displayed uncommonly narrow.

Clinical Case 20081003 - General overview 


  • Patient: Male, 36 year old.
  • Clinical symptoms: ileal J-pouch-anal anastomosis alter total colectomy for UC, two years ago.
  • 1 month ago, surgical drainage of a perianal abscess.
  • External opening at 3 o’clock, 3 cm away from anal margin.

Clinical Case 20080102 (bis) - General overview


  • Patient: Male, 36 years old.
  • Clinical symptoms: light anal irritation and discharge of bloody drain. Patient had an anal abscess ten months ago.
  • Clinical inspection of the anus, and endorectal ultrasound were performed.

 

Abbreviations: EAI: Internal anal sphincter; EAE: External anal sphincter.

 

An introduction of contrast agent was done, as a method to enhance fistula.  The tract begins one and a half centimetre outside the anal opening and crosses the external sphincter muscle an ends the space between the internal and external muscle (see image 1). The fistula track on the left lower quadrant can be seen.

Diagnosis: intersphincteric fistula.  
Treatment: fistulectomy.
