The ATLAS Of Ultrasound Sonography

The ATLAS Of Ultrasound Sonography belongs to the U-Surgery.com Basic Course Collection and it is available through the www.u-surgery.com and www.educational-surgery.com sites. It has been produced with the collaboration of Dr. Albert Navarro and Dr. José Antonio Pando, from the Hospital Mutua de Terrasa, in Barcelona, both of them well known Spanish Colorectal Surgeons. This work also has been possible thanks to the work of Dr. José Manuel Ramírez, from the Hospital Clínico Lozano Blesa, in Zaragoza, and Dr. Javier Fabra, PhD Computer Engineer from the Department of Computer Science and Systems Engineering at the University of Zaragoza.

During the last decade, endoanal ultrasonography has become a very helpful tool not only in the study of rectal tumors but also as a diagnostic complementary aid for benign anal disease, mainly in patients with fecal incontinence and fistula. In fact, nowadays endoanal ultrasound, which is an easy-to-perform and well-tolerated technique, is considered an indispensable means for evaluating the anatomy of the anal canal and has replaced other, more uncomfortable techniques.


The coming of anal endosonography to the study of fecal incontinence has probably been the most important improvement in the management of these patients during the last decades. It is of most importance for selecting a treatment option, and establishing the actual situation of the anal canal sphincter, not only by the recognition of a muscle tear but also by identifying the location and amount of tissue involved.


Anal endosonography has been shown to clearly define the nature of the anal sphincters and permit the identification of perianal sepsis with accuracy. Three-dimensional ultrasound is particularly useful in anal sepsis and it is of great value as a complementary tool for preoperative clinical assessment of fistulas in ano by providing clear anatomical detail of the fistula track in relation to the anal sphincters.


Moreover, the mobility of the apparatus permits endosonic examination at the patient couch side, in the operating theatre or at the out patient clinic.


The images shown in the ATLAS are from our own experience, all of them come from BK equipment (Leopard 3535 and Profocus). We have used the BK viewer for the 3D pictures.


The only purpose of this Atlas is to offer the reader a collection of careful selected images to be used as a starting point to everyone whishing to utilise this valuable tool in proctological assessment.
